چکیده

Fournier’s Gangrene is a life threatening urosurgical emergency and a synergistic gangrene of scrotum. It is a rapidly progressive and devastating form of necrotizing fasciitis of perineal, perineum and genital area associated with high mortality and morbidity. Radical surgical debridement, broad spectrum antibiotics, negative pressure wound dressings and hyperbaric oxygen therapy are considered to be the cornerstone of treatment and contribute to improved outcomes.. We present a case of 57 year old gentleman with Fournier gangrene whose case was complicated by presence of rare multiple bacteria and was successfully managed with early diagnosis, prompt measures and use of hyperbaric oxygen. Introduction Fournier’s Gangrene is acute, rapidly progressive and potentially fatal, infective necrotizing fasciitis affecting external genitalia, perineal and perianal region 2, . It is characterized by a synergistic, polymicrobial necrotizing fasciitis leading to the thrombotic occlusion of small subcutaneous vessels and the development of gangrene. The disease affects all ages, both sexes and is more common in males. The predisposing factors which may contribute to development of disease are Diabetes Mellitus (DM), alcoholism, malignancy, immunosuppression, liver or renal disease. It is associated with high mortality rates, hence urgent, aggressive and effective treatment of this condition is imperative to prevent fatal outcome.

Hyperbaric oxygen as adjuvant therapy in the management of necrotizing fasciitis.

BACKGROUND Necrotizing fasciitis (NF) is an uncommon but serious infection of fascia and skin associated with considerable morbidity and mortality. One modality proposed for improving the outcome of this condition is hyperbaric oxygen (HBO) therapy. This is a form of medical treatment that involves intermittent inhalation of 100% oxygen under pressures exceeding the atmosphere. The aim of this ...

Treatment of Fournier's Gangrene with Combination of Vacuum-Assisted Closure Therapy, Hyperbaric Oxygen Therapy, and Protective Colostomy

Fournier's gangrene is a rare process which affects soft tissue in the genital and perirectal area. It can also progress to all different stages of sepsis, and abdominal compartment syndrome can be one of its complications. Two patients in septic shock due to Fournier gangrene were admitted to the Intensive Care Unit of Emergency Department. In both cases, infection started from the scrotum and...

A case of Fournier's gangrene after hydrocelectomy

An uncommon case of Fournier's gangrene following hydrocelectomy is described. A 78-year-old male with no remarkable previous history, who underwent hydrocelectomy in another hospital, developed Fournier's gangrene 15 days later. The patient required wide aggressive surgical debridement, hyperbaric oxygen chamber and broad-spectrum antibiotic coverage. Afterwards the patient was referred for pl...
